Guardrails and Safety

Guardrails are systems that constrain LLM behavior to ensure safe, reliable outputs and prevent unsafe actions. This chapter covers input validation, output filtering, prompt injection defense, action safety, hallucination mitigation, and reliability patterns for production systems.

Interview Questions

Q: How do you prevent hallucination in a production RAG system?

Strong answer: Multi-layer approach:

1. Retrieval quality: - High-quality retrieval is the first defense - If we retrieve wrong context, model will hallucinate - Use reranking to ensure relevance

Q: How do you protect an LLM application from prompt injection?

Behavioral guardrails: - Confidence thresholds: escalate to human if uncertain - Refusal patterns: graceful decline for out-of-scope requests - Disclosure: clearly identify as AI when appropriate

Fallback chain:

Primary LLM -> Backup LLM -> Canned responses -> Human escalation

Monitoring: - Log all guardrail triggers - Track guardrail trigger rates - Alert on high block rates (may indicate attack or model issue) - Sample blocked conversations for review - User satisfaction tracking

The balance is: enough guardrails to be safe, not so many that the bot is useless. Tune thresholds based on the risk profile -- financial services tighter than casual chat.
